Description
Wake County is seeking proposals for a one-time $500,000 competitive grant to support acquisition, design, renovation, or construction of a medical respite facility serving individuals experiencing unsheltered homelessness. The facility must provide at least 20 medical respite units and be operational by January 2028. Eligible applicants may be for-profit or nonprofit organizations, including collaborative or joint-venture applicants with a designated lead entity that owns and develops the project and is financially responsible for property-related debt. The proposed facility must provide short-term housing and residential care, including meals, personal care, wellness services, wound care, medication management, medical appointment transportation, case management, healthcare connections, benefits navigation, housing navigation, and discharge planning. Applicants must demonstrate development and financial capacity, relevant organizational and medical respite experience, supportive-service partnerships, a sustainable 10-year operating plan, property ownership or a binding purchase contract, and compliance with accessibility, environmental, building, health, safety, HMIS, reporting, Bridge to Home, Continuum of Care, and medical respite best-practice requirements. Proposals must include organizational, project, supportive-services, and financial narratives; source-and-use and operating budgets; and required organizational and financial documentation where applicable.
