Description
The South Carolina Department of Social Services is establishing a qualified providers list for supervised independent living services for eligible young adults ages 18–21 participating in the Extension of Foster Care Program. Providers must furnish safe, stable, community-based apartment housing, including furnishings, utilities, maintenance, laundry access, food support, transportation assistance, and housing-related life-skills instruction. The service scope includes admissions and readiness assessments, individualized transition and support plans, weekly face-to-face case-management contacts, crisis intervention, service navigation, health and well-being coordination, monthly workshops and social activities, and support for education, employment, parenting, transportation, and independent living. Providers must maintain required staffing, security cameras, reporting, background-check, information-security, insurance, and program-monitoring capabilities. The maximum stated rate is $137 per apartment-style living day, and awards may be made to all responsive and responsible offerors.
