Description
The City and County of Denver, through the Child Welfare Division of Denver Human Services, is seeking applications from qualified agencies and organizations to establish a pool of potential providers for out-of-home placement services for children and youth from birth through age 18, or up to age 21 when placed in custody before their 18th birthday. Service categories include child placement agency services, child residential habilitation programs, group home or group center care, independent living, psychiatric and qualified residential treatment facilities, residential child care facilities, shelter care, and transition or home-based after-care. Providers must maintain current licenses and accreditation; use trauma-informed, culturally responsive, and inclusive service models; provide basic 24-hour care and related medical, dental, educational, case management, behavioral health, and family-engagement services; satisfy applicable Families First Prevention Services Act and Medicaid authorization requirements; conduct required background checks; provide language access; maintain legally compliant records; and use secure technology for data collection and record-keeping. Monthly and progress reporting on youth condition, family circumstances, education, incidents, and treatment-plan progress is also required.
