Description
Alameda County Behavioral Health Department is establishing a pool of Services as Needed contracts with eligible licensed Adult Residential Facility and Residential Care Facility for the Elderly operators to provide Housing Support Program services to adults age 18 and older living with serious mental illness in Alameda County. Services include care and supervision in a home environment, care coordination, transportation, health-care and community-service linkages, daily-living support, medication-related support, behavioral supervision, housing-stability assistance, and support toward more independent living. Providers may propose multiple levels of care, ranging from basic board and care through specialized services such as oxygen management, CPAP support, ostomy, catheter and wound care, diabetic and insulin care, dementia care, hospice, and continuous bedside care. Reimbursement is monthly per client and varies by level of care from $1,141 to $8,494; the County has allocated $7,194,413 per contract year for fiscal year 2026–2027, but does not guarantee any minimum or maximum award amount or scope of services. Responses must use the Bid Response Packet and include the required signed bidder forms, exceptions and clarifications, SLEB information, exclusion-list background-check attestation, proposal narrative, current adult residential or elderly-care licensure, staff list and schedule, and applicable safety and operating plans. The proposal narrative describing staff training, experience, and plans to serve individuals with serious mental illness is limited to two pages. The response must be submitted as a single PDF proposal plus a completed Excel staff schedule, with the bidder's name, by email.
