Description
The City of Hammond is soliciting applications for Program Year 2027 Emergency Solutions Grant funding. Eligible activities include emergency shelter renovation, rehabilitation, conversion, maintenance, operations, rent, repairs, security, utilities, food, furnishings, essential services, transitional housing, homelessness prevention assistance, and limited administrative costs. Applicants must explain their mission, proposed use of funds, eligible populations, projected residential and nonresidential service levels, homelessness documentation procedures, participant involvement, Continuum of Care participation, budget, timeline, matching funds, and organizational capacity. The application requires original signatures and supporting documentation, including financial information, a conflict-of-interest form, board and organizational information, an IRS tax-exempt determination, insurance documentation, program literature, and a current client intake form. Funding may be provided for a period not exceeding three years, is subject to federal and local requirements, and does not guarantee continuation in subsequent action plans.
