Description
Arvada Fire Protection District is soliciting proposals from qualified, appropriately licensed medical providers with occupational-health expertise and knowledge of National Fire Protection Association firefighter-health guidelines. The selected provider will perform pre-employment, fit-for-duty, and annual physical examinations for firefighters, Fire Marshal's Office safety employees, fleet mechanics, maintenance employees, Urban Search and Rescue personnel, and hazardous-materials employees. Services include drug and alcohol screening and may include physiological measurements, urinalysis, chest X-rays, electrocardiograms, musculoskeletal and tuberculosis screening, disease testing and vaccinations, metabolic and lipid testing, Department of Transportation physicals, and heavy-metals testing as needed. The provider must supply equipment and materials; manage scheduling, examination-result summaries, medical-record maintenance and secure storage, follow-up notifications, referral coordination, and occupational-health advice and consultation at no additional cost. Proposals should describe the firm, service approach, team experience, three comparable fire-department references, compensation and fees, conflicts of interest, and insurance coverage. Required insurance includes commercial general liability of at least $1 million and workers' compensation at statutory limits. Services must be available during normal Monday-through-Friday business hours, with examinations scheduled within five business days for hiring-process employees and within 30 days for incumbent employees requiring annual examinations.
