Description
The County of Fresno is soliciting proposals for field-based, housing-focused street outreach and CalAIM Housing Navigation services for individuals and families experiencing unsheltered homelessness. Services include proactive outreach, homelessness diversion, housing assessments and applications, Coordinated Entry System integration, assistance with Medi-Cal enrollment and benefits, transportation or transportation arrangements, referrals and warm handoffs to housing and behavioral health resources, CARE Act petitioning, and MH/SUD/OUD screening and referrals. Contractors must document activities in HMIS and applicable managed-care platforms, bill eligible Housing Navigation services to Medi-Cal managed care plans, and provide monthly performance and fiscal reporting. The program is divided into geographic zones, with awards made by zone. Metro zones require service seven days per week from 7:00 a.m. to 7:00 p.m.; rural zones require weekday coverage from 8:00 a.m. to 5:00 p.m. and at least four hours each Saturday. Proposals may cover up to five zones and must include implementation, staffing, service-delivery, outcomes, collaboration, and detailed cost narratives, along with completed budget templates. The initial term is three years with two optional one-year renewals. The solicitation identifies maximum five-year budgets by zone and requires bidders to provide staffing match and leverage CalAIM revenues.
