Description
The State of Vermont Agency of Human Services, Department of Disabilities, Aging & Independent Living, in collaboration with the Department for Children and Families Office of Economic Opportunity, is seeking applications for a specialized emergency shelter model serving medically vulnerable adults experiencing homelessness. The selected grantee or grantees will operate one or two shelters with approximately 20 to 25 beds each, in facilities meeting applicable fire, building safety, accessibility, and privacy requirements. Required services include clinical oversight, case management, personal care, skilled nursing, meals as needed, laundry and cleaning, transportation options, and coordination with outside care providers. Applicants must address shelter operations, staffing, partnerships, leveraged Medicaid and Medicare services, performance measures, reporting, HMIS and coordinated entry participation, and local government approval. The total estimated funding is $1,801,500 for the period January 1, 2027, through June 30, 2028; administrative costs may not exceed 15% of the total award. Property acquisition, new construction, mortgage costs, and supplanting billable services are ineligible.
