Description
The New Hampshire Department of Health and Human Services, Division of Public Health, is seeking one vendor to design and deliver targeted improvement training and technical assistance focused on Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) measures for New Hampshire Critical Access Hospitals. The work includes developing strategies for participating hospitals to improve selected HCAHPS measures, supporting positive patient experiences and organizational improvements, and facilitating quarterly learning collaborative meetings to share best practices, address challenges, and review progress. Responses must address the vendor’s healthcare-provider training experience, quality-improvement project experience, capacity and staffing, relationships with Critical Access Hospitals, and approach to annual work plans. The submission requires separate Technical Response and Cost Proposal electronic documents. The Technical Response must include the specified appendices and resumes for key personnel; the Cost Proposal must include completed deliverable pricing by State Fiscal Year and, if applicable, a Federally Negotiated Indirect Cost Rate Agreement.
