Description
The Minnesota Department of Human Services, through its Homelessness, Housing and Support Services Administration, is soliciting proposals for one-time capacity-building grants supporting new or existing Housing Support Program providers serving individuals receiving Medical Assistance home and community-based services. Eligible projects may strengthen administrative and organizational capacity, improve connections to housing benefits, enhance housing transition activities, and develop regional or collaborative models that expand access to stable, community-integrated housing. Grantees must complete an approved work plan, collaborate with partners as appropriate, participate in technical assistance and evaluation activities, collect and report performance data, and submit narrative and financial reports. Eligible costs include administration, staffing and operations, and certain housing-transition expenses subject to stated limits. The total appropriation is $1,250,000, with no maximum award amount or limit on the number of awards. Applications must use the required forms and statements and be submitted through the HHSSA Foundant online grants management system.
