Description
The New Hampshire Department of Health and Human Services, Division of Medicaid Services, is seeking one vendor to establish and operate a statewide Medicaid Medication Risk Reduction Initiative. The selected vendor must employ or contract with New Hampshire-licensed clinical pharmacists to identify high-risk beneficiaries, conduct comprehensive medication reviews, prepare medication safety reports, and provide actionable recommendations to beneficiaries, medical providers, and Medicaid managed care organizations. The initiative addresses medication-related problems, adverse drug events, polypharmacy, over-prescribing, opioid-related risks, and behavioral-health medication risks. The vendor must provide technology capable of analyzing Medicaid pharmacy encounter and claims data, use algorithms to identify medication-risk opportunities, integrate medication safety reports with provider and managed-care technology systems, support patient engagement and medication adherence, and demonstrate measurable reductions in total cost of care. Responses must include technical answers, staffing information, resumes, culturally and linguistically appropriate services materials, an application requirements response, and a separate deliverables cost sheet. The anticipated total funding is $33,553,391, with a contract effective date of January 1, 2027, an end date of December 31, 2029, and potential extension through September 30, 2031.
