Description
The New Hampshire Department of Health and Human Services, Division of Public Health, is seeking one contractor to support chronic disease prevention, screening, management, cancer prevention, cardiovascular health, diabetes, arthritis, asthma, and related preventive-health programs. The scope includes partner and stakeholder engagement; evidence-based intervention implementation; healthcare and community-system coordination; professional education and training; continuing-education support; educational and patient materials; communications; meeting and registration logistics; technical assistance; administrative support; and program evaluation. The selected vendor must address five mandatory technical questions covering relevant experience, ability to perform the full scope, chronic-disease and screening expertise, proposed approach, and organizational capacity. The response must include staffing information, an organizational chart, staff qualifications and resumes, separate technical and cost proposals, and completed budget materials for each state fiscal year. The anticipated contract has an effective date upon Governor and Executive Council approval, ends June 30, 2028, and may be extended for up to two additional years. The solicitation identifies $750,000 in total funding for innovative pilot projects across the program areas, but does not state a total contract award amount.
