Description
Alameda County Behavioral Health Department seeks licensed Adult Residential Facility and Residential Care Facility for the Elderly operators to join a Services as Needed contractor pool for its Housing Support Program. Contractors will provide ongoing care and supervision, housing stability support, care coordination, transportation, community integration, health-care linkages, medication-related support, daily living assistance, and individualized supplemental services to adults age 18 and older living with Serious Mental Illness in Alameda County. Services may range from basic board and care through specialized Level 4 care, including oxygen management, wound care, dementia care, hospice, and continuous 24-hour bedside care. The County allocated $7,194,413 per contract year for FY 2026–2027, but guarantees no minimum or maximum award amount. Reimbursement will be based on monthly per-client rates ranging from $1,141 to $8,494, depending on the level of care. Applicants must maintain applicable ARF or RCFE licensure, demonstrate capacity and experience serving individuals with Serious Mental Illness, satisfy level-specific qualifications, complete required staff screening and training, and provide an emergency safety plan and plan of operation when proposing Levels 2, 3, and/or 4 services. The contract term will generally be one to three years and may be renewed subject to funding, performance, and program priorities.
