Description
Alameda County Public Health Department's Office of Violence Prevention is seeking one qualified provider for a Hospital-Based Violence Intervention Program supporting the Alameda County Violence Reduction and Recovery Initiative. The selected provider will deliver trauma-informed, culturally responsive, survivor-centered services for victims of community gun violence and their families, including bedside engagement, tailored case management, mentoring, life-skills development, wraparound care, violence interruption, street outreach, community education, group therapy, referrals, home visits, and ongoing support. Services must be provided at Alameda Health System-Highland Hospital, UCSF Benioff Children's Hospital of Oakland, Sutter Health Eden Medical Center, and Washington Hospital, with capacity to serve up to 50 individuals annually who are age 34 or younger. The provider must demonstrate at least three years of relevant violence-prevention experience, experience serving vulnerable Alameda County communities and accessing and evaluating HIPAA-compliant data, appropriate cultural and linguistic competency, qualified personnel, implementation capacity, and the ability to submit quarterly progress reports using Results-Based Accountability measures. The County anticipates one award with funding of up to $500,000; the contract term is 2 years and 3 months, with a possible one-year extension. Proposals must include the County's Bid Response Packet, budget form, service and implementation narratives, deliverables and reporting plan, references, and required certifications and forms.
