Description
Alameda County seeks one qualified contractor to implement a comprehensive, evidence-informed Hospital-Based Violence Intervention Program for residents affected by community gun violence. Services must be provided at four designated trauma hospitals and include bedside engagement, trauma-informed case management, mentoring, life-skills development, wraparound care, violence interruption, street outreach, community education or group therapy, follow-up support, referrals to community resources, home visits, and assistance addressing social determinants of health. The program must serve up to 50 individuals annually who are age 34 or younger and support patients and families treated for gun-violence-related injuries. The contractor must demonstrate at least three years of relevant violence-prevention/intervention experience, including work with vulnerable Alameda County communities and HIPAA-compliant data. Proposals must describe relevant experience, key personnel, proposed services, implementation plans, deliverables and reporting, data collection and continuous quality improvement, budget details, and three references. The contract will be funded at up to $500,000, with services continuing through March 31, 2029; an additional one-year extension may be mutually agreed upon.
