Description
The City of Fremont is seeking proposals for a 12-month temporary shelter program serving unhoused households at partner hotel locations. Proposers may offer either service coordination only or full program operations and service coordination. Service coordination includes referrals and enrollment support for employment, benefits, healthcare, housing, and shelter transitions; vital-document readiness; housing problem-solving; flexible housing and client-support assistance; weekly client meetings; collaborative case coordination; and HMIS data entry and assessments. The full-operations option additionally includes intake and enrollment, unit preparation and inspections, coordination with hotel management, daily and overnight site monitoring, enforcement of program and site rules, discharge planning, and exit procedures. Proposals must address the provider profile, recent relevant experience, project approach, estimated costs and staffing, organizational capacity, and recommendations for improving deliverables and client support. The proposal must include a cover letter and four exhibits: Scope of Services, Billing Rates, Statement of Qualifications, and Terms of Service Agreement, submitted together as one PDF.
