Description
The City of Oxnard Housing Department is soliciting proposals for the Traveling Home Street Outreach Services Program. The selected contractor will conduct proactive, person-centered, trauma-informed outreach to individuals experiencing unsheltered homelessness; assess immediate needs and barriers; develop service plans; and provide referrals and assistance related to emergency, interim, and permanent housing, shelter, health and behavioral health services, public benefits, identification, transportation, and other community resources. The contractor must coordinate with the City, homeless-service providers, medical and behavioral health providers, housing and shelter providers, Continuum of Care organizations, and other community partners. Responsibilities also include identifying and reporting public health, sanitation, and safety concerns; coordinating around encampment-related activities; maintaining programmatic and financial records; and submitting monthly performance reports. The proposal must address organizational experience, assigned staff, relevant project experience, outreach and case-management approach, coordination, performance measurement, compliance, and cost. The program is funded with CDBG-CV funds and includes applicable federal grant requirements.
