Description
San Andreas Regional Center seeks an agency to operate a Family Home Agency using a Family Teaching Model in a duplex configuration. A family home provider lives in one residence, while up to three individuals with developmental disabilities live in the separate, attached or contiguous residence. The agency will recruit, assess, approve, train, and monitor providers; match individuals with homes; and provide ongoing support, respite and emergency backup, placement transitions, and quality assurance in accordance with applicable California Title 17 requirements. The proposal must describe the program model, staffing and provider recruitment and training, individual planning and outcomes, health and safety procedures, transportation, continuous quality improvement, and financial resources. Applicants must submit the specified proposal appendices, including financial and obligations statements, qualifications and references, a narrative program plan, and proposed start-up milestones if applicable. Appendix F budget/rate materials are conditional and must be requested from SARC if applicable. The selected applicant will also be required to complete a program design after award and maintain appropriate general and professional liability insurance.
