Description
The State of Delaware, through the Department of Health and Social Services' Division of Developmental Disabilities Services, is seeking qualified vendors to provide home and community-based services for individuals with intellectual and developmental disabilities, including brain injury, autism spectrum disorder, and Prader-Willi Syndrome. Service categories include assistive technology, behavioral consultation, community participation and transition, crisis stabilization, day and residential habilitation, personal care, respite, remote supports, supported living, supported employment, nursing consultation, benefits counseling, financial coaching, accessibility adaptations, and specialized equipment or supplies. Vendors must be qualified through the DDDS Provider Authorization Committee for the services proposed and must submit the required vendor forms, non-collusion statement, exceptions form, confidentiality form, business references, qualified provider letter(s), Supplemental Questionnaire, Delaware business license or IRS 501(c)(3) determination letter, and certificate of insurance. Vendors must accept DDDS-established fee-for-service rates; rate negotiation is not permitted. Subcontracting is permitted with prior State approval, but the prime vendor remains responsible for performance. The contract is for one year with four one-year extension opportunities.
