Description
The State of Alaska Department of Health and Social Services, Division of Senior and Disabilities Services, is soliciting proposals for one grant award to operate assisted living homes providing Senior Residential Services in remote Interior Alaska communities. The program serves seniors and adults with disabilities who cannot live independently but can reside safely in a supervised setting with minimal assistance. Required services include screening and admission, assessment and individualized care planning, skilled nursing, medication management, personal care, meals, housekeeping, activities, community integration, transportation and resource access, monitoring, documentation, incident reporting, complaint resolution, care coordination, and compliance with applicable assisted living, Medicaid waiver, and reporting requirements. Applicants must demonstrate relevant experience, staffing and credentials, facility licensure, Medicaid provider status or an application plan, financial and administrative capacity, emergency response procedures, community support, partnerships, and referral coordination. The anticipated funding is $205,000 for FY2027 and $615,000 for the 33-month program, with a required 10% match. The application must include a proposed first-year budget, timeline, Planned Services form, evaluation plan, agency and staffing information, facility photos, emergency response plan, community-support documentation, and partnership documentation.
