Description
This five-year service agreement will pilot an alternative housing model in which users rent units made available by SHDM and receive support intended to promote autonomy, personal development, and social inclusion. The service provider must assist users in developing or acquiring the confidence and skills needed for everyday living and provide the support and assistance required for their daily routines. The provider must deliver continuous services 24 hours per day, seven days per week, including domestic and daily-living support. Sante Quebec Centre-Ouest-de-l'Ile-de-Montreal Universitaire will assign a lead worker to assess users' functional abilities and performance and a quality worker to oversee service quality.
