Description
Leelanau County is seeking proposals from qualified agencies to provide one or more services to eligible residents age 60 and older, including in-home respite care, personal care, medication management, homemaking, routine foot care, and transportation to medical appointments. Services are intended to support seniors' independence, safety, and quality of life while they remain in their homes and communities. More than one agency may be selected for award. Respondents must provide qualified and trained personnel, maintain applicable licenses, conduct personnel background checks, and comply with confidentiality, infection-control, incident-reporting, and insurance requirements. Proposals must address the scope of services, qualifications and experience, customer references, personnel resumes, service approach, potential project problems, pricing and expenses, certifications and assurances, responsiveness checklist, and acceptance of contractual conditions. The contract is planned as a two-year term with two automatic one-year renewal options.
