Description
The New York State Office of Mental Health is soliciting grant proposals from eligible nonprofit supportive-housing providers to operate six Teams to Promote Aging in Place in OMH Supportive Housing. Each interdisciplinary team must include an OMH supportive-housing case manager, a local office for aging case manager, a home-care professional, and at least one mental health or substance-use/gambling peer professional. Teams will serve residents age 55 and older who are at risk of losing community tenure, as well as younger residents with complex medical conditions requiring home care. The program requires person-centered assessment and care planning, enhanced care coordination and cross-system navigation, connections to social-determinants-of-health and social-engagement resources, enhanced home-care support, paid peer services, wrap-around assistance, data collection, and participation in training and learning collaboratives. Applicants must partner with local aging and home-care organizations and submit required commitment letters. OMH intends to make six awards, including at least two for rural counties, at $300,000 annually for five years per award, or $1.5 million per award. Applicants must be prequalified nonprofit organizations by the submission deadline. The updated schedule sets the proposal deadline at December 1, 2026, at 2:00 PM Eastern Standard Time, with anticipated award notification on January 14, 2027.
