Description
The Department of Veterans Affairs is seeking a contractor to provide contracted residential services for up to three homeless Veterans per day through the Health Care for Homeless Veterans program. The scope includes room and board, food and hygiene necessities, laundry, timely admissions, therapeutic and rehabilitative services, case management, transportation to medical appointments, supportive services, and assistance with permanent housing, employment, recovery, and independent living. The facility must maintain required Pennsylvania licenses, permits, certifications, accreditations, and occupancy approvals; provide qualified personnel and 24/7 emergency availability; maintain a safe, drug- and alcohol-free environment; and accommodate Veterans with varied medical, mental-health, substance-use, and mobility needs. The procurement is unrestricted and seeks a five-year firm-fixed-price IDIQ/requirements contract with annual task orders and daily pricing for three beds. Technical capability, past performance, and price will be evaluated. Quotations must describe the facility location and bed capacity, priority-population capabilities, personnel, meals, transportation, community engagement, case management, and therapeutic or rehabilitative activities, and must include applicable licenses, permits, certifications, accreditations, and key-personnel information. A past-performance questionnaire may be provided for up to three examples. The contractor must comply with inspection, reporting, privacy, records-management, medical-liability-insurance, and business-associate requirements.
