Description
The County seeks a provider to deliver outpatient specialty mental health services to Medi-Cal beneficiaries ages 5–21 and transitional-age youth ages 16–25. Services include assessment, case management, intensive care coordination, therapeutic behavioral services, and individual, family, and group therapy, supported by six full-time-equivalent clinicians. The provider must use evidence-based practices, maintain a minimum caseload of 25 beneficiaries per clinician, and provide services at a fully state-certified Medi-Cal site within the county. The provider will coordinate with the County, support culturally and linguistically appropriate care, maintain required clinical documentation and outcome measures, and participate in quarterly contract monitoring. The proposal requests a project approach, firm and team qualifications, three recent agency references, and a fee proposal with hourly rates by licensing type and service, a budget narrative, and itemized costs for bundled services. The proposed contract term is three years, with a possible single one-year extension.
