Description
Santa Cruz County is soliciting a contractor to implement Hepatitis C virus linkage and navigation services during fiscal year 2026–2027. The scope includes screening priority populations; coordinating confirmatory laboratory testing, medical visits, treatment initiation, prescription pickup, medication adherence, and treatment completion; helping clients obtain medical benefits and identify medical homes; providing nutritional supplements when appropriate; distributing client incentives; addressing treatment barriers; and coordinating care with County public health staff and treating providers. The contractor must collect outcome data, complete HCV assessments, seek releases of information, support care coordination, and meet monthly with the County HCV program. Respondents must provide qualifications, experience, references, licenses or permits, sample pricing, a service list, approach, pricing, compliance responses, and required County exhibits and signature materials. The stated budget is $45,841. The intended contract runs through June 30, 2027, with one one-year extension option.
