Description
Miami University and Ohio University Purchasing Group is requesting proposals from a qualified contractor to perform annual testing, inspection, reporting, and minor maintenance of designated backflow prevention assemblies at Miami University facilities. The contractor must provide labor, supervision, equipment, tools, transportation, and materials; verify proper operation; complete required testing documentation; submit reports to the University and the City of Oxford's electronic reporting system; notify the University of failed assemblies; provide repair recommendations and cost estimates; retest repaired devices as required; and tag each tested assembly with the test date, tester name, certification number, and result. The attached bid tab identifies the assemblies, tags, locations, models, serial numbers, buildings, and manufacturers. Proposals must address the RFP requirements in order, include all costs, identify exceptions or deviations, provide at least two similar-project references with contact information, include sustainability information and responses to applicable sustainability questions, and include the completed and signed Proposal Offering Form. The contract is intended to begin November 1, 2026, with the application functional and fully installed by November 15, 2026, and service continuing through November 14, 2029; extensions of up to three additional one-year periods may be mutually agreed upon. Required insurance includes automobile liability, general liability, workers' compensation, and cyber liability when applicable.
