Description
The University of Wisconsin-Madison, through its HIV Care and Prevention Program, seeks an experienced community-based agency to provide housing assistance, housing case management, emergency financial assistance, and housing-related legal services for people with HIV living in Dane County. Services should focus on eviction prevention, linkage to existing housing resources, short-term housing assistance, rental and utility assistance, housing assessments and plans, landlord-tenant disputes, discriminatory housing practices, fair-housing barriers, and related legal needs. Criminal defense and class-action lawsuits are excluded. The anticipated annual service population is approximately 70–90 clients. The selected contractor must coordinate with UWHIV medical case managers and local housing partners, provide monthly client-level reporting, maintain auditable case and expenditure records, participate in required care-coordination and programmatic meetings, and comply with applicable protected-health-information requirements. Year 1 funding is expected not to exceed $335,000, including $50,000 for emergency financial assistance and $25,000 for legal expenses; applicants must submit a five-year budget and budget narrative. The anticipated performance period is April 1, 2027, through April 30, 2032, subject to funding availability and annual renewal.
