Description
The Township of Hamilton is soliciting qualifications and proposals from individuals or firms to provide professional veterinarian services. The selected respondent must demonstrate the professional, administrative, and financial capability to perform the required services and must submit information addressing qualifications, key personnel, technical competence, experience, availability, servicing-office location, conflicts of interest, disciplinary history, and compensation or fee schedules. The proposal must include the required proposal form and supporting forms and certifications, including ownership disclosure, addenda acknowledgment, insurance acknowledgment with general liability certificate, affirmative-action compliance notice, Iran disclosure, Russia-Belarus certification, non-collusion affidavit, New Jersey Business Registration Certificate, references, and W-9. At least three similar-project or contract references are required, including at least two municipalities. The contract term is January 1 through December 31, 2027, with award anticipated at the first or second Council meeting in January 2027.
