Description
The Iowa Department of Health and Human Services is soliciting proposals from Iowa county boards of health or their designated local public health agencies to serve as district-level Public Health Lead Entities. Selected entities will coordinate, plan, and strengthen delivery of communicable and infectious disease control, environmental public health, and chronic disease and injury prevention services within one or more Iowa HHS Districts. Applicants must be located within the district or a contiguous district and may apply for no more than two contiguous districts. The scope includes districtwide assessment and planning, provider-network development, stakeholder coordination, public engagement, data collection and reporting, partnership building, and administration of public health funds. Required work products include a Phase 1 project plan, districtwide public health system analysis, interim district public health system plans, coordination reports, and ongoing Phase 2 implementation and monitoring. The anticipated contract term is January 1, 2027, through June 30, 2032, consisting of an 18-month initial period followed by up to four one-year extensions. Funding includes up to $4,140,000 for up to seven Phase 1 awards and an anticipated $9,204,082 for Phase 2 activities, subject to appropriations and Agency discretion.
