Description
The State of Montana, through the Department of Public Health and Human Services, is seeking a vendor to assist with developing a statewide Community Health Aide Program (CHAP) under the Rural Health Transformation Program (RHTP). The scope requires offerors to address each item in the Scope of Services document and submit responses through the state's electronic procurement system. Required response materials include Business Information, a detailed response to the Scope of Services, Offeror Qualifications, a cost proposal, and responses concerning contract exceptions, confidential information, nondiscrimination against firearm entities or trade associations, artificial intelligence use, pre-existing partnerships, and related mandatory requirements. The engagement is structured as a lump-sum service over five years and involves federal funding, associated reporting obligations, insurance requirements, and federal form requirements for the awarded contractor.
