Description
The Delaware Division of Public Health is soliciting proposals from private organizations to deliver health programs and services funded by the Delaware Health Fund for Fiscal Year 2028, covering July 1, 2027, through June 30, 2028. The RFP is issued pursuant to 16 Del. C. § 137(i) and prioritizes tobacco prevention and cessation and cancer prevention, detection, and treatment. Proposals must include a transmittal letter, cover letter, statement of need, alignment with Health Fund priorities, program description, measurable outcomes and evaluation plan, organizational capacity, budget and budget narrative, certifications and attachments, specified forms, workplan and project cost templates, and required organizational records. The contract term is one year, contingent on appropriation, and evaluation will consider program need, alignment with priorities, program design, measurable outcomes, organizational capacity, and budget reasonableness.
