The Hawaiʻi State Department of Health’s Communicable Disease and Public Health Nursing Division seeks one provider to administer a pilot integrating Community Health Workers into public health nursing services in rural communities. The provider must recruit, employ, and deploy 6.0 FTE Community Health Workers and at least a 0.5 FTE Project Manager across Maui, Hawaiʻi, Kauaʻi, and Honolulu Counties; manage payroll, benefits, recruitment, screening, equipment, connectivity, travel reimbursement, training, and workforce performance; and coordinate administrative oversight with daily clinical direction from Department of Health public health nurses. The scope also includes evidence-informed health education, outreach, resource navigation, follow-up support, secure cloud-based performance tracking, monthly and quarterly reporting, and a final nine-month evaluation report. The solicitation identifies a probable maximum funding amount of $658,000, requires at least three years of relevant Community Health Worker workforce and training experience, and requires general liability insurance of at least $1,000,000 per occurrence/$3,000,000 aggregate and automobile insurance of at least $1,000,000 per occurrence.
