Description
Oakland County is soliciting proposals for the development, implementation, expansion, or enhancement of warm hand-off and recovery programs serving individuals impacted by substance use disorders. Eligible activities include peer recovery coaching, recovery navigation, support groups, family recovery supports, adolescent and young-adult programming, transportation assistance, recovery housing and housing stability supports, recovery community center development, workforce and organizational capacity expansion, and integrated linkages among treatment, healthcare, housing, justice, education, and community providers. Projects must serve individuals who live or work in Oakland County, address barriers to accessing services, demonstrate evidence-based or promising practices, and show a clear connection to reducing opioid-related harm. Applicants must submit organizational information, at least three references, a project and technical proposal, a completed logic model, measurable outcomes, sustainability and partnership information, a budget and budget narrative, a project director resume or biography, a current W-9, and required certifications and supporting documentation. Oakland County anticipates an initial allocation of up to $3.36 million, but the number and size of awards will depend on available settlement funds; multi-year projects may be proposed, although funding commitments are made one year at a time.
