Description
Iowa State University of Science and Technology is seeking an electrohydraulic shockwave platform for its athletics program. The requested solution may be new, used, refurbished, or a previous demonstration unit and must include pricing for the equipment, annual software costs, and freight. Respondents must provide total cost of ownership for three-year and five-year terms, including annual cost breakdowns, one-time and ongoing costs, and any annual price-increase caps. The response must address the solution's therapeutic coverage zone, documented biological effects, FDA 510(k) clearances, support services, implementation timeline, training, accessibility, and security requirements. Required attachments include a detailed quote, implementation timelines and details, technical documentation, a service-level agreement, and applicable terms and conditions in editable Word format. Three references with contact information are required, with higher-education references of similar size and scope preferred.
