Description
The New York State Office of Mental Health is soliciting proposals from eligible 501(c) nonprofit agencies that provide OMH supportive housing to establish Teams to Promote Aging in Place in OMH Supportive Housing. Each program must serve residents age 55 or older who are at risk of losing community tenure, or younger residents with complex medical conditions requiring home care. The interdisciplinary team must include an OMH supportive-housing case manager, a local Office for the Aging case manager, a home-care professional, and at least one certified or provisionally certified mental-health or substance-use/gambling peer professional. Awardees must provide person-centered assessments and care planning, enhanced care coordination and cross-system navigation, home-care support, peer services, connections to community resources addressing social determinants of health and social engagement, wrap-around assistance, data collection, and participation in training and learning collaboratives. Six awards are anticipated, including at least two for rural counties, with funding of $300,000 annually for five years per award; $9 million is available in total. Applications must include a proposal narrative and supporting attachments in a combined PDF, a five-year Excel operating budget, proof of local-government-unit notification, partner commitment letters, and required certifications, and must be submitted through the Statewide Financial System.
