Description
The selected provider will deliver on-site medical and behavioral health services for families and pregnant people seeking shelter, including health screenings, targeted medical evaluations, episodic and urgent care, immunizations, contraception counseling and provision, health education, referrals, and crisis intervention. Services include behavioral health and substance-use screening, de-escalation, overdose-prevention services, and coordination with hospitals and community providers. The provider must maintain clinic coverage daily from 8:00 a.m. to midnight, provide 24/7 phone or email coverage for urgent needs, and supply qualified clinical staff, medical supplies, and infectious-waste disposal. The scope also includes patient-record and confidentiality practices, staff training, data reporting, and program coordination. The anticipated contract term is three years, with one three-year renewal option. The stated maximum funding is $4,942,950 for the initial three-year term, with a maximum annual budget of $1,647,650. The proposal must include a completed budget summary and supporting program, staffing, financial, reference, and medical-linkage materials.
