Description
Iredell County seeks a qualified provider to staff and operate its existing employee wellness clinic for eligible employees, retirees, and dependents. The core scope includes onsite primary and preventive care delivered by clinical personnel under a physician medical director; routine examinations, diagnosis and treatment, laboratory and point-of-care testing, prescriptions, care coordination, chronic-condition management, health coaching, and patient education. The provider must also support wellness initiatives, including biometric screenings, health-risk assessments, and administration of the County’s incentive program. Integrated 24/7 telehealth and behavioral-health access are valued components, and expanded laboratory access is requested for members who cannot readily visit the clinic. Occupational health, workers’ compensation, physical therapy, drug and alcohol testing, and related services are optional at the County’s election. Proposals should describe the service-delivery and staffing model, implementation and transition plan, clinical leadership and credentials, security and compliance practices, reporting capabilities, and performance commitments. The response must address the evaluation criteria and include at least four recent references for similar projects, a company-standard proposed service contract, the completed E-Verify and Iran Divestment Act certifications, and pricing using the provided cost structure. The initial contract term is three years, with up to three one-year renewals.
