Description
The New York City Department of Social Services/Human Resources Administration Adult Protective Services program is seeking four qualified not-for-profit providers to deliver Community Guardian Program services across all five boroughs. Services include court-appointed guardianship, comprehensive assessment and individualized service planning, case management, medical and behavioral-health coordination, housing stabilization, legal and court services, fiduciary and financial management, benefit advocacy, emergency response, client visits, reporting, and compliance with court and agency requirements. The solicitation has three competition pools: Behavioral Health, Mixed Population, and Geriatric/Dementia. Separate proposals are required for each category. The anticipated base term is three years, from July 1, 2027, through June 30, 2030, with one three-year renewal option. Four contracts are anticipated, with total anticipated base-term funding of $43,464,958.26, subject to a 25 percent allowance and final negotiations. Proposals must be submitted through PASSPort, and proposers must have an approved HHS Prequalification Application. Subcontracting is permitted with prior agency approval, but one lead agency is required.
