Description
The City seeks one or more providers to administer a program that prevents eligible households from becoming homeless and helps households facing or experiencing homelessness find alternatives to shelter or return to housing. Services include strengths-based problem-solving and case management, mediation, housing search and referrals, connections to mainstream services, transportation, and flexible financial assistance for eligible housing, utility, moving, and related costs. Assistance is generally capped at $7,500 per household for each program component, subject to City approval for exceptions; prevention rental assistance may be provided for up to three months, and eligible prevention rental arrears may cover up to six months. Providers must coordinate referrals through the Coordinated Entry System, enter client data into HMIS, submit a relevant HMIS data-quality report, and provide regular performance reports and participate in oversight and community coordination. Proposals must describe the proposed approach, schedule, staffing and experience, projected households served, and costs. The City anticipates making approximately $500,000 available.
