Description
The State of Vermont is seeking one or more contractors to provide onsite workplace medical monitoring and testing. Required services include occupational hearing testing; qualitative and quantitative respirator fit testing; respirator and silica medical clearance; OSHA respiratory medical questionnaires; height, weight, and vital-sign screening; pulmonary function testing; chest X-rays with B-reading; physical examinations; TB screening; employee medical recordkeeping; and Hepatitis B vaccination administration and follow-up titers. The contractor must provide qualified and appropriately certified or licensed personnel, calibrated equipment, climate-controlled testing environments, reporting and employee result documentation, confidential recordkeeping, and onsite visits at multiple Vermont locations. The response must include a cover letter, technical response, references for at least three similar customers from the last 12 months, a reporting-documentation sample, pricing in the provided Price Schedule, and a completed Certificate of Compliance. Electronic submission requires one email containing one digitally searchable PDF attachment with all bid components.
