Description
The City seeks a vendor to provide and administer a secure, web- and mobile-accessible employee wellness platform, including implementation, configuration, data integration, eligibility management, participant support, account management, and reporting. Platform capabilities include health risk assessments and PCP visit attestations, personalized health reports, biometric and wearable-device integration, wellness challenges, communications and educational content, and incentive and reward tracking with payroll reporting. The program serves eligible employees and their eligible spouses or qualified domestic partners, with separate participant eligibility segments and controls over access to segment-specific activities and incentives. The vendor must support implementation and open enrollment, provide ongoing technical and participant support, and meet the City’s privacy, security, accessibility, and data-protection requirements. The solicitation requests a five-year contract with a one-year extension option. The response includes a technical proposal, completed pricing workbook and vendor questionnaire, references, required certifications and offer forms, and a performance-bonding ability letter; the technical proposal checklist calls for a single PDF, while the pricing workbook must be returned in Excel and PDF.
