Description
The New York City Department for the Aging seeks qualified health agencies, social support agencies, and housing entities to operate Naturally Occurring Retirement Community Supportive Service Programs (NORC SSPs). Programs must provide five core services: case assistance, case management, healthcare assistance, healthcare management, and health promotion. Services include benefits and resource assistance, ongoing care coordination, health consultations and screenings, chronic-condition support, and wellness activities. Providers must operate from an eligible, accessible residential site and meet specified staffing, service-level, partnership, outreach, and emergency-preparedness requirements. Optional supplemental services may include education and recreation, transportation, meals, friendly visiting, shopping assistance, housekeeping, personal care, and telephone reassurance. The solicitation anticipates 36 awards across five competition pools. A separate proposal is required for each site and competition pool. The anticipated contract term is July 1, 2027, through June 30, 2030, with renewal options of up to three additional years. Annual budgets are guided by participant counts, and contractors must provide matching funds equal to at least 50% of annual NYC Aging funding, subject to stated housing-match exemptions. Subcontracting is permitted with agency approval. Proposals and required prequalification applications must be submitted through PASSPort; vendors need an account and an approved HHS Prequalification Application.
