Description
Salt Lake County Human Resources is soliciting proposals for comprehensive employee medical examination and testing services. The scope includes Department of Transportation physical examinations, law-enforcement pre-employment physicals, physical and mental fitness-for-duty examinations, tuberculosis testing for existing law-enforcement employees, and pre-employment, random, and reasonable-suspicion drug and alcohol testing for law-enforcement, DOT, and safety-sensitive employees. Testing may include urine, hair, breath-alcohol, vision, spirometry, electrocardiograms, audiometry, laboratory panels, and other specified examinations. The contractor must use licensed medical examiners listed in the Federal Motor Carrier Safety Administration National Registry for DOT examinations; comply with applicable Utah, federal, HIPAA, and medical-practice requirements; provide standardized reports and retain supporting documentation for at least seven years; and meet stated turnaround targets. Proposals must address qualifications, relevant experience, staffing, locations, certifications, methodology, customer service, reporting, billing, privacy, and pricing. The initial agreement term is four years, with an option for one additional two-year period, and the County may award to multiple acceptable proposers.
