Description
Salt Lake County Human Resources is soliciting proposals for medical examination and testing services. The scope includes approximately 150 DOT physicals, 150 law-enforcement pre-employment physicals, 1,200 tuberculosis tests, and pre-employment and random drug and alcohol testing for approximately 750 DOT employees and 420 non-DOT employees annually, along with reasonable-suspicion testing and other potential fitness-for-duty examinations. Required services include physical, mental, cognitive, psychological, and physical fitness-for-duty evaluations; DOT/FMCSA examinations conducted by licensed medical examiners listed in the FMCSA National Registry; law-enforcement examination testing such as laboratory panels, tuberculosis testing, HIV and hepatitis antibody baselines, drug screening, vision, spirometry, EKG, and audiometry; and 10-panel drug and alcohol testing using urine, hair, and breath alcohol testing. Proposals must address staffing, locations, scheduling, reporting, customer support, pricing, relevant experience, references, privacy, regulatory compliance, and performance guarantees. The contract is expected to have an initial four-year term with one optional two-year period.
