Description
San Mateo County seeks qualified providers to deliver home health and non-medical in-home support to conservatees and other vulnerable adults. Services may include personal care, homemaker and chore assistance, meal preparation, shopping and errands, medication reminders and observation, escort and accompaniment, respite, safety monitoring, home organization, and communication support. The County may award contracts to multiple providers and use them on a rotation basis. Providers must be able to respond to referrals, deliver individualized services under County-authorized plans, maintain qualified and screened personnel and backup coverage, coordinate with County staff and care teams, and document and report service delivery, incidents, and invoices. The proposal requires responses addressing the provider’s qualifications, approach, staffing, service coverage, performance monitoring, invoicing, and references, as well as applicable licenses and insurance evidence.
