Description
The County of San Diego is seeking proposals from firms to provide authorized Home and Community-Based Care Services for clients referred by Aging & Independence Services. Lot 1 covers the North Coastal, North Inland, and North Central regions. Required services include homemaker assistance, non-medical personal care, in-home respite care, and transportation with escort. Services support older adults and adults with disabilities who are at risk of institutionalization and may include household tasks, activities of daily living, caregiver relief, and escorted transportation to medical and social-service appointments. The County intends to award two contracts for the lot: one primary provider and one alternative provider. The anticipated term is a one-year initial period with three one-year options, with an initial term beginning May 1, 2027. The estimated maximum available funding for Lot 1 over the full four-year term is between $1,839,200 and $1,936,000. Proposals must include program and implementation approaches, quality-control procedures, experience and references, financial information, pricing, required licenses, a linguistic and cultural diversity plan, and small-local-business utilization information.
