Description
The County of San Diego, through Aging & Independence Services, is soliciting proposals for Lot 2, covering the East, Central, and South regions, to provide authorized Home and Community-Based Care Services to older adults, adults with disabilities, and family caregivers referred through County programs. Required services include homemaker assistance, non-medical personal care, in-home respite care, and transportation with escort services, including transportation to medical and social-service appointments. The County anticipates awarding two contracts for the lot: one primary provider and one alternative provider. The initial term is one year with three one-year option periods. Proposals must address program design, implementation, quality control, staffing, relevant experience, references, financial stability, licensing, pricing, insurance, subcontracting, and cultural and linguistic diversity. The estimated maximum available funding for Lot 2 over the full four-year term is $2,340,800 to $2,464,000.
