Description
The County seeks an organization to operate and staff the Betty Ehart Senior Center and White Rock Senior Center and provide services for older adults and other eligible participants. The scope includes congregate and home-delivered meals, transportation, health and wellness and social programming, volunteer opportunities, adult day care, respite care, home-based services, and care coordination. The contractor must provide qualified personnel and resources, coordinate with community and service-provider organizations, and enroll in and actively use the Unite Us referral platform. The contractor will also provide monthly service data and quarterly progress reports, support participant feedback and program evaluation, and make reasonable efforts to obtain supplemental funding. Proposals must include a narrative response to the specified proposal components, a cost proposal covering potential contract years, and the required forms and supporting materials. The contract term may be up to seven years.
