Description
The Area Agency on Aging of Palm Beach/Treasure Coast, Inc. is seeking organizations to serve as the Community Care System Lead Agency for separate Community Care Service Areas within Planning and Service Area 9. The selected lead agencies will provide or coordinate services for eligible clients under the Community Care for the Elderly, Home Care for the Elderly, and Alzheimer's Disease Initiative programs. Proposals are requested separately for Indian River, Martin, Okeechobee, and St. Lucie counties, and for Palm Beach County Service Areas A and B. The anticipated service period begins July 1, 2027. Proposal format and instructions are available from the issuing agency's website. Submission requires one original hard copy, one additional hard copy, and an electronic copy on USB drives, enclosed in a securely sealed envelope or container.
